Connecting Healthcare Providers and Patients

At Trelle Health, we understand the importance of the connection between healthcare provider and patient and the importance of patient-centred care. Unfortunately, navigating the healthcare system is often difficult for patients who are not familiar with the many facets of such a large and complex structure.

Trelle Health provides nurse consultation services within the scope and standards of nursing practice to support and improve hands-on care delivery. We specialize in rheumatology, specifically osteoporosis, osteoarthritis and rheumatoid arthritis care but recognize there are often co-morbidities when managing chronic disease conditions. We work with care providers and their patients, emphasizing quality of life when living with a chronic disease, such as osteoporosis, osteoarthritis or rheumatoid arthritis. We focus on helping patients meet their education needs so they can understand and navigate of the health care system. This education supports them to make informed decisions about their care and management of their disease in cooperation with their healthcare provider.

Our consultant, Gwen Ellert, has a lifetime of personal experience living with a chronic disease and resulting medication decisions. Having being in and out of health care facilities for various joint replacements and revisions, she knows the issues patients and families face. Being a registered nurse, she also appreciates the points of view of those healthcare providers who are working diligently to care for the patients in a hectic and sometimes confusing environment.
